Application Process

If you are interested in applying to Lake Forest Academy, please complete the following steps. Please note that all forms may be downloaded from the right side of this page.
1. Complete the Online Application
Complete the online General Application as soon as you decide to apply to Lake Forest Academy. Submission of this form initiates the application process. Please plan to apply well before the January 31 deadline.

2. Schedule an Interview
All Lake Forest Academy applicants must be interviewed by a member of the admission office. The Office of Admission must receive the online General Application before an interview can be scheduled. Please call 847-615-3267 to schedule an interview on campus (preferred) or to schedule a telephone interview.

3. Student Essays
Respond to Questions 1-3. Essays must be the applicant’s own work, hand-written, and signed.

4. Recommendations
Recommendations can now be completed online.  This option is only available for domestic applicants.
For your application to be considered complete, you must complete the following:

• English Teacher Recommendation
• Math Teacher Recommendation
• School Recommendation


Our preferred method is to have recommendations completed online using schoolrecs.com; it is “green” and reduces the administrative burden for all parties – you, the recommenders and us. It notifies your recommenders of due dates, lets you know when your recommendation has been completed, and can transmit recommendations electronically. You can even include background information when you request a recommendation to assist your recommender in personalizing your recommendation.

Alternatively, Please download the recommendation forms from the Admissions Downloads Forms on the right side of this page and give them to the appropriate school official and teachers. The School, English, and Math Recommendations must be completed by current school officials and teachers.  The recommendations must be returned to the applicant in sealed and signed envelopes. Once all recommendations are collected, mail them to the Office of Admission.

5. Transcripts
Parents should complete the Confidential Records Release Form and submit it directly to the applicant’s current school (please also send a copy to Lake Forest Academy’s Office of Admission). The school should send official tran scripts directly to the Office of Admission. We require transcripts for the past two consecutive years of schooling.

6. Testing
Applicants to grades 9, 10, and 11:
Please register the applicant to take the Secondary School Admission Test (SSAT). For more information on SSAT testing dates and sites, and to register for the test, please visit www.ssat.org. Please keep in mind the test must be taken prior to the application deadline.
 
Lake Forest Academy’s SSAT school code is 4450.
 
Applicants to grade 12 and Post-graduate candidates:
Take the PSAT, SAT, ACT-Plan, or ACT test prior to January. For more information about these tests, visit www.collegeboard.com.

7. Athletic Supplement (Optional)
f applicant has any specific athletic interests that he/she would like the admissions committee to know about, please complete the Athletic Supplement.

8 Arts Supplement (Optional)
If applicant has any specific artistic interests that he/she would like the admissions committee to know about, please complete the Arts Supplement.
Information for International Applicants
In addition to the steps above, international applicants must also submit the following:

1. Copy of passport face page and U.S. Visa, if applicable

2. Statement of Financial Support

3. Test of English as a Foreign Language (TOEFL) Scores
If English is not the first language of the student applying, or if they have not been at an English-speaking school for at least four years, they must also take the Test of English as a Foreign Language (TOEFL).

For information on or to register for the TOEFL, please go to www.ets.org/toefl. The TOEFL code for Lake Forest Academy is 8161, and official scores should be sent directly to Lake Forest Academy. Lake Forest Academy does not accept SLEP scores. Please contact the admission office if you have questions about the TOEFL.
